A survey released this week by enterprise software firm OutSystems should give pause to any CXO who considers their organization’s AI deployment a story of progress. The 2026 State of AI Development report, which drew on responses from nearly 1,900 IT leaders across 20-plus industries globally, found that 96% of organizations are already using AI agents in some capacity, and 97% are actively exploring system-wide agentic strategies. The shift from pilots to production, the report concludes, is no longer underway. It has happened.

India, notably, is ahead of the curve. Among APAC markets surveyed, India reported some of the highest concentrations of advanced and expert agentic AI capability. Financial services and technology sectors globally showed the highest rates of production deployment. By the standard metrics that enterprise technology watchers use to measure maturity, this looks like a success story.

It is not, or not entirely. Buried in the same data is a finding that deserves considerably more attention in boardrooms: 94% of organizations report concern that AI sprawl is increasing complexity, technical debt, and security risk. Only 12% have implemented a centralized platform to manage it. Most enterprises, the report finds, are running agents across fragmented environments, governed by approaches that vary by team and region, with no coherent architectural authority over the whole.

The gap between 96% and 12% is the actual story here. Enterprises have deployed far faster than they have governed, and that asymmetry has quietly become an operational liability.

What Agentic AI Actually Means at Scale

To understand why this matters, it helps to be precise about what agentic AI is and what it is not. An AI agent, in the enterprise context, is not a chatbot or a content generation tool. It is a system capable of autonomously executing multi-step workflows, making decisions within defined parameters, and adapting its actions in real time based on incoming data. It does not wait for a human to prompt it at each step. It runs.

Gartner has projected that 40% of enterprise applications will include task-specific AI agents by the end of 2026, up from fewer than 5% in 2025. The OutSystems report suggests this trajectory is conservative. Thirty-one percent of respondents already describe AI as integral to their development practices, and another 42% have embedded it into specific phases of the software development lifecycle. Generative AI-assisted development has emerged as the leading deployment method in markets including India and Australia.

The operational implications of this scale are significant. Fifty-two percent of organizations now rely on what the report calls a “human-on-the-loop” model: agents operate with reduced direct oversight, and humans maintain supervisory rather than transactional control. The systems are, in effect, making consequential decisions continuously, across functions, without a human approving each one.

That is a reasonable operating model when the systems are well-defined, auditable, and fail gracefully. The OutSystems data suggests most enterprise deployments are none of these things.

The Fragmentation Problem

Thirty-eight percent of organizations globally report mixing custom-built and pre-built agents, creating AI stacks that are difficult to standardize and impossible to secure uniformly. Teams are building agents independently, with governance approaches that vary by project. The result is what the report calls AI sprawl: a proliferation of autonomous systems operating without coherent oversight, accountability structures, or failure protocols.

The tools for agent governance exist. Calling this a technology failure misreads the situation entirely. The problem is organizational. Enterprises that moved quickly on deployment did so because deployment has a visible champion: the CTO or the Head of AI who can point to productivity gains, reduced cycle times, or faster software delivery. Governance has no equivalent champion. It is slower, less glamorous, and its value becomes visible only when something has already gone wrong.

When something goes wrong with an AI agent, it tends to go wrong at scale and at speed. An agent managing customer communications that receives incorrect instructions could send erroneous information to thousands of customers before a human notices. An agent embedded in financial workflows that operates on stale or corrupted data does not produce one bad decision. It produces thousands. The compounding nature of agent errors is precisely what makes governance so consequential, and precisely why the 94% figure in the OutSystems report should be treated as an alarm rather than an observation.

What Indian Enterprise Leaders Should Examine

India’s position in this data is worth examining carefully. Being among the most advanced in APAC for agentic AI capability is a genuine achievement, reflecting years of investment in AI talent, infrastructure, and organizational appetite for adoption. Advanced capability without governance architecture, however, is exposure, not a competitive advantage.

Indian enterprises operate in a regulatory environment that is still developing its frameworks for AI accountability. The Digital Personal Data Protection Act, 2023 establishes obligations around data processing, but specific guidance on autonomous AI systems, agent-generated decisions, and associated liability remains limited. Organizations building agent infrastructure now are doing so in a window of regulatory ambiguity that will not remain open indefinitely. The governance frameworks built today will either position enterprises well for regulatory compliance tomorrow, or create costly remediation work when clearer rules arrive.

There is also a procurement dimension. As Indian enterprises sell AI-enabled products and services to global clients, especially in financial services, healthcare, and legal sectors, the governance of their underlying AI systems is becoming a diligence question. Clients in regulated markets are increasingly asking not just whether a vendor uses AI, but how that AI is controlled, audited, and corrected when it produces bad outputs. Enterprises that cannot answer this question clearly are already losing deals they may not know they have lost.

The Governance Questions That Boards Should Be Asking

For CXOs reading this, the OutSystems data raises a specific set of questions that are not currently on most board agendas. Four stand out as foundational, and most organizations cannot honestly answer any of them.

Inventory

How many agents are you running, where are they deployed, what data do they access, and who, if anyone, is accountable for their outputs? Most organizations lack a complete picture. Before any governance framework can be built, this inventory must exist.

02 Accountability
Ownership

When an AI agent causes harm (a financial error, a compliance breach, a data exposure), who is responsible? The team that built it? The team that deployed it? The vendor behind the model? Without explicit accountability structures, the answer is contested, slow to resolve, and wrong by design.

03 Compliance
Auditability

For agents operating in regulated functions like finance, HR, legal and compliance, every consequential decision should be traceable. The inputs, the reasoning path, the output, and the human oversight applied should be logged and retrievable. Most enterprises cannot meet this standard today.

04 Architecture
Failure Design

Agents that encounter unexpected inputs or ambiguous situations need defined failure modes: when to escalate to a human, when to halt, when to ask for clarification. Building these protocols is not an engineering afterthought. It is a governance requirement that must be specified before deployment, not discovered during an incident.

Speed and Control Are Not Opposites

There is a version of this argument that enterprises have heard before and learned to dismiss: slow down, add process, reduce risk. The OutSystems data is not making that argument, and neither is this piece. The enterprises that will build durable advantage from agentic AI are not those that deploy most cautiously. They are the ones that deploy with coherent architectural control.

OutSystems CEO Woodson Martin put it simply in the report’s accompanying statement: the challenge facing enterprises is building a stable architectural foundation for coordinating complex intelligent systems. That is a structural problem, and structural problems require structural solutions. A governance framework that is bolt-on, team-level, or review-as-needed does not qualify. A centralized platform for managing agent deployment, monitoring agent behavior, and enforcing policy across the stack does.

Only 12% of enterprises have built that. The remaining 88% are running a governance deficit that grows larger with every agent they deploy. For Indian enterprises that are among the most advanced in the region, the urgency of closing that deficit is proportional to the scale of the capability they have already built.

The agents are running. The question governance has not gotten around to is whether anyone is actually in charge of them.
